OSHA regulations require annual fit testing for all employees who use respirators. Additional testing is required whenever there are changes that might affect respirator fit, such as significant weight changes (10% or more), dental work, facial surgery, or scarring in the seal area. Some employers choose to test more frequently to ensure ongoing protection.
Qualitative fit testing relies on the wearer's sensory detection of test agents (taste or smell) to identify respirator leakage, while quantitative testing uses specialized equipment like PortaCount to measure the actual concentration of particles inside and outside the respirator, providing a numerical fit factor. Quantitative testing offers more precise results and is required for certain respirator types.
OSHA's Respiratory Protection Standard (29 CFR 1910.134) requires employers to implement a comprehensive respiratory protection program that includes workplace hazard assessment, appropriate respirator selection, medical evaluations, fit testing, training, proper use procedures, cleaning and maintenance protocols, and regular program evaluation. All components must be documented and maintained.
A typical respirator fit test requires 15-30 minutes per employee, depending on the type of test and number of respirator models being tested. Qualitative tests generally take 15-20 minutes, while quantitative tests may require 20-30 minutes due to equipment setup and calibration. The total time also depends on how many different respirator models need to be tested for each individual.
OSHA regulations prohibit facial hair that comes between the sealing surface of the respirator and the face or that interferes with valve function. Employees with beards, stubble, or other facial hair that crosses the respirator sealing surface cannot achieve a proper seal and will not pass a fit test. Only respirators that don't rely on a face seal, such as certain PAPRs with loose-fitting hoods or helmets, may be used by employees with facial hair.
If an employee fails a fit test, the employer must provide a different respirator model, size, or style that may provide a better fit. The employee must be successfully fit tested with the new respirator before using it in the workplace. If a proper fit cannot be achieved with any tight-fitting respirator, the employer may need to provide a loose-fitting powered air-purifying respirator (PAPR) that doesn't require a face seal, or implement other protective measures.
The medical clearance process begins with the employee completing OSHA's mandatory medical questionnaire (or equivalent). A physician or other licensed healthcare professional (PLHCP) reviews the questionnaire and may conduct additional examinations based on the employee's responses. The PLHCP then provides a written recommendation regarding the employee's ability to use the respirator, including any limitations or follow-up evaluations needed. This process must be completed before fit testing.
