Complete the OSHA Respirator Medical Questionnaire Online

ProTrain lets employees complete the respirator medical questionnaire from their own device and routes it for licensed healthcare professional review. Employers receive the appropriate evaluation status and released result—not the employee's medical answers.

What is the OSHA respirator medical questionnaire?

OSHA's Appendix C contains the Respirator Medical Evaluation Questionnaire used as part of the medical-evaluation process under the respiratory protection standard. An employer may use the questionnaire or an initial medical examination that obtains the required information.

The purpose is to give the licensed healthcare professional the information needed to evaluate whether the employee can safely use the intended respirator under the expected work conditions.

Medical evaluation comes before fit testing

  1. 1

    Medical evaluation

    Employee completes the medical-evaluation process.

  2. 2

    Fit testing

A fit test is completed when required for the selected tight-fitting respirator.

  3. 3

    Respirator use

The employee uses the respirator under the employer's respiratory protection program.

A medical evaluation is not a fit test. ProTrain provides the online medical-evaluation workflow. QuickCare's in-person fit-testing service is separate.

Medical information goes to the clinical workflow

Your employer does not receive your questionnaire answers or clinical reasoning.

Review happens after submission

Automated screening runs the moment an employee submits. It can flag a response for review, but it never decides that someone is not cleared — a licensed clinician makes every clearance decision.

What does the employer receive?

Employers never see the employee's health information. They see the status of the evaluation and what to do next—not the questionnaire, the answers, or the clinical reasoning.

Employers never see the questionnaire responses, diagnoses, medications, risk flags, or the clinician's reasoning — only the status below and what to do next.

What happens after the questionnaire is submitted?

  1. 1

    Submission

The employee submits the completed medical questionnaire.

  2. 2

    Screening and review

Automated screening runs at submission and can identify responses that require additional review. A licensed clinician makes the clearance decision.

  3. 3

    Follow-up when needed

Some submissions can be decided from the questionnaire. Others require additional information before a decision can be made.

  4. 4

    Employer-safe result

The employer receives the appropriate released clearance result and status without receiving the employee's medical answers.

Frequently asked questions

Is the OSHA respirator medical questionnaire the same as a fit test?

A respirator medical evaluation is not a fit test. OSHA requires the medical evaluation before an employee is fit tested; QuickCare performs fit testing separately and in person.

Can the questionnaire be completed online?

Yes. Employees complete the questionnaire on their own device, at any hour, usually in one sitting. No appointment is needed for this step.

Can my employer see my answers?

No. Employers never see health information. They see the status of the evaluation and what to do next — never the questionnaire, the answers, or the clinical reasoning.

What happens if my answer needs more review?

Some submissions are cleared on the clinician's review of the questionnaire alone. Others need follow-up information before a decision can be made.

Does ProTrain do fit testing too?

No. ProTrain provides the online medical-evaluation workflow only. QuickCare's in-person fit-testing service is separate.
