A work-related injury or illness that meets specific criteria set by OSHA (under 29 CFR 1904) and must be recorded on the employer's OSHA 300 Log. These criteria include death, days away from work, restricted work activity or job transfer, medical treatment beyond first aid, significant injury or illness diagnosed by a physician or other licensed health care professional (PLHCP), or loss of consciousness.
